Episiotomies are something that come up time and time again when we discuss what women hope to avoid during labour! Whilst there are practical ways to avoid an episiotomy, such as being in an upright position, Birthing in water, or perineal massage in pregnancy- it’s important to know what an episiotomy actually is and why it might be used.
For our Mother’s generations, episiotomies were a pretty routine part of labour care!! (Along with shaving and enemas!!) thankfully though, this is a thing of the past! Episiotomies are now only used (in the U.K.) when there’s a clinical need. Usually this is because baby might need to be born more quickly, to create space if you are having an instrumental birth or importantly as a preventative measure of more severe tearing.
If you are having an episiotomy you’re given local anaesthetic (like if you were having a tooth taken out), and it should always be something discussed with you beforehand. As with anything in birth- you have the final say! An episiotomy will need stitching with dissolvable stitches after birth. Warm baths, ‘padcicles’, lots of rest and pouring warm water over them as you go for a wee can all help relieve discomfort and promote healing.
