This patient presented to me after sustaining a nasal fracture (#brokennose
) during a football game. Upon examination, his nasal septum was also found to be fractured and deviated to the left, completely obstructing his nasal airway and preventing him from breathing properly.
I performed an open reduction of nasal bone fractures and a #septoplasty
, effectively correcting his cosmetic deformity and relieving the nasal obstruction.
The "after" photo was taken 7 days post-operatively, immediately following nasal splint removal. The swelling is still significant as expected at this point. It will begin to subside over the next couple of weeks, but will take several months for the majority of it to resolve.

Deep plane facelift
From the patient's perspective, a "facelift" is probably one of the most misunderstood of all the procedures that I do.
While almost all patients would prefer a mini-lift, three-day lift, weekend-lift, S-lift, etc., the reality is that these types of minimally invasive facelifts are generally either disappointing in terms of the result or the longevity. They are good for a young (e.g. 42-year-old) with mild jowl laxity, but otherwise, a deep plane lift is much more effective.
In the attached video, I show an intra-operative deep plane facelift from last week. It demonstrates the initial subcutaneous elevation and the exposure of the SMAS layer which is a continuation of the platysma. The majority of surgeons (68%) stop here and use sutures to plicate or fold the SMAS in an attempt to "tighten" the face. This is completely ineffective for the midface and for the lower neck. Furthermore, asymmetric distribution of tension leads to an uneven, unnatural, pulled look.
Today, many surgeons are starting to use a subSMAS approach where they elevate the SMAS for a centimetre or two. This is slightly better, but as the video demonstrates, without complete release of the zygomatic, mandibular, and masseteric ligaments, the midface and neck will not fully release. I generally will dissect approximately 10 cm along the jawline in a deep plane as well as fully release the midface and divide the ligaments sharply over the zygomaticus to gain maximal mobility of the SMAS flap. The majority of the branches of the facial nerve are in close proximity to where I am working, but they are completely safe when dissected carefully.
Contrary to popular belief, when bleeding is controlled meticulously and the tissues are handled gently, this does not increase swelling or downtime for patients. It does however, result in a vastly more powerful lift, and one that redrapes the midface, jowls, and neck in a smooth, uniform vector.
Finally, for most patients, I combine full facial fat grafting to optimize their result. There is no tension on the skin. The incisions are closed with 5-0 and 6-0 sutures (not staples). The hairline is carefully analyzed and respected
