At the risk of not making this account simply a collection of highlight reels, I wanted to show you the other side to MS. I had a GP appt this morning and walked to town and back for it. I am now absolutely exhausted and so I have spent the afternoon having a 1hr30 nap and then resting in bed since. Although I have been able to enjoy getting out the house more and seeing friends, this afternoon reminds me that I am recovering from a pretty intense treatment of lemtrada and that I must listen to my body when it needs rest. Just a reminder to everyone that rest is good and okay 😊.

Given the alarming epidemic of obesity and its related diseases worldwide, there is a renewed focus upon elucidating the mechanisms underlying the regulation of energy metabolism, mitochondrial function and control of cellular redox balance. The relationship of energy metabolism, redox chemistry and generation of reactive oxygen species (ROS) is easily apparent at the mitochondrial level in which superoxide is generated as a consequence of the electron transport chain activity and Kreb's cycle activity. Exciting, new data indicate that nutrient intake, energy metabolism and ROS are also linked at the nuclear level via the nuclear factor E2-related factor 2 (Nrf2)/antioxidant response element (ARE) pathway.
Nrf2 is commonly recognized to augment cellular defense against elevated oxidative damage. At its simplest, the Nrf2 transcription factor resides in cytoplasm until increases in cellular oxidative stress cause it to translocate to the nucleus. Nrf2 binds to the cis-acting enhancer ARE sequence (core sequence: TGAG/CNNNGC) present in promoters of genes that code for proteins necessary for glutathione synthesis and electrophile detoxification . Transcriptional activation of ARE-mediated genes requires that Nrf2 heterodimerizes to other basic leucine zipper proteins, including Jun (c-Jun, Jun-D and Jun-B) and small Maf (MafG, MafK and MafF) [2,3]. Additionally, the nrf2 gene contains two ARE-like sequences in its promoter so that Nrf2 can autoregulate itself and potentially sustain ARE-mediated gene expression longer [3,4]. Newer data detailed below suggest that Nrf2 indirectly affects gene expression through inhibition of the p300 acetylase . Recent research shows that the Nrf2/ARE pathway plays an important role at the intersection of energy metabolism and macronutrient intake. Genomic, proteomic and physiologic data from several laboratories implicate Nrf2 in energy metabolism and may be a target for treatment of obesity-related disorders. In this work, we will review the current data detailing the role of the Nrf2/ARE pathway as it relates to energy metabolism and energy storage, as well as identify areas for further research.

Epstein-Barr virus (EBV) infection is a strikingly common virus in the human population worldwide. Most people acquire EBV in early childhood, experience no symptoms or only a brief, mild cold-like illness, and remain infected throughout their lives while remaining asymptomatic. When infection first occurs in adolescence or young adulthood, EBV can lead to a syndrome of infectious mononucleosis characterized by prolonged fever, sore throat, swollen lymph nodes and fatigue. This syndrome, also known as “mono” or the “kissing disease,” generally resolves with rest and only rarely causes serious complications.
A recent study conducted by scientists at National Institute of Allergy and Infectious Diseases (NIAID) reports that the Epstein-Barr virus (EBV), also increases the risks for some people of developing seven other major diseases. These diseases are lupus erythematosus, multiple sclerosis, rheumatoid arthritis, juvenile idiopathic arthritis, inflammatory bowel disease, celiac disease, and type 1 diabetes.
When EBV infects human immune cells, a protein produced by the virus — EBNA2 — recruits human proteins called transcription factors to bind to regions of both the EBV genome and the cell’s own genome. Together, EBNA2 and the human transcription factors change the expression of neighboring viral genes. Researcher were surprised to find that nearly half of the locations on the human genome known to contribute to lupus risk were also binding sites for EBNA2. These findings suggest that EBV infection in cells can actually drive the activation of these genes and contribute to an individual’s risk of developing the disease.
Autoimmune diseases are difficult to treat and can result in debilitating symptoms. Studies like this are allowing reseachers to untangle environmental and genetic factors that may cause the body’s immune system to attack its own tissues.
Take home message: Better understanding of the complex causes of autoimmunity might lead to better treatment and prevention options in future.
